Score Report Form

If you are a Referee - Don't use this form - Use the Referee Report Form
This form must be filled out by the League's Town Representative or designated individual only
and must sent after each slate of games has been completed.
Submit form no later than the evening of the day of the games.

Failure to submit this report in a timely manner will result in League fines
and hinder timely posting of scores and standings.

If there are more than 4 games that you are reporting, then send a separate report for the additional games.
